1) Visit the ‘Reporting a Complaint About a Health Care Facility’ page on the Indiana State Department of Health’s (ISDH) website and download their complaint form.

The form will ask you to fill in a number of items related to the negligent facility, including your relationship to the resident, a description of concerns, as well as your contact information.

It is important to note that you may also submit the form anonymously. However, the ISDH form states that if you do provide your contact information, they will keep your name confidential.
